Submit proposal as an email with attachment files to: info@chim-nwp.org

Documents that must be submitted with the email as attachment files are as follows:

1. Completed and signed proposal form in pdf format
2. Surveyed map of donated land with dimensions in pdf format
3. At least one picture of donated land in jpg or jpeg format.
